Line of Duty Death Wall addition Your assistance will be required to fill out the attached form so that the information about the deceased will be accurate. Please read the guidelines to determine if any of your deceased members fits the criteria. The file below is a Adobe file for you to print off and complete. Click on the blue letters Line of Duty Death application form If you do not have adobe acrobat reader you can down load a free copy at http://www.adobe.com/products/acrobat/readstep2.html DECEASED MEMBER GUIDELINES The deceased member must have died in an "ACTIVE DUTY" status with a Fire Department in the State of Missouri, the interpretation will include: at work at the station, in response to, on an alarm, or returning from and includes, providing service (such as Public Relations). This will cover members of all fire departments to include a Department of Public Safety, where the individual was working as a fire fighter. If there are any questions as to an individual meeting the criteria, do not hesitate to give me a call. The deceased's individual does not have to be a member of the Fire Fighters Association of Missouri to be placed on the Memorial Wall. Any and all fees will be waived for the departed individual, family and fire department. Please take a few minutes and fill out the attached form as completely as possible and return in the self-addressed envelope. To find additional information about the Fire Fighters Memorial or the Fire Fighters Memorial Foundation please do not hesitate to call or contact any Officer or District Representative of the Fire Fighters Association of Missouri, or the Missouri State Fire Marshal's Office. Our efforts to complete the memorial continue Even as we unveil the Memorial we are continuing to collect funds to pay for the site. You can help through sponsorship of a brick in the Walk of Honor. Check below for more information. With the competition of the Memorial (phase 2) we will now move on to phase 3. A 10,000+ square feet structure to house a Museum. We will need all of Missouri's fire service to help us in this effort. We will also look to our states corporation to lend a hand. If you have contact with an organization that may be able to help please let us know and start talking to them now. September 19th 2001 the members of New York Fire Department accepted our statue of a kneeling fire fighter in prayer as a gift from the fire service of Missouri. The statue is heroic size equaling 1 ½ life size. It measures over six feet tall and is over 3000 pounds, it was in route to Missouri and was being held in customs at Kennedy airport on September 11th the day of the tragedy. The Fire Fighters Memorial Foundation in cooperation with Dave DeCarlo of Matthews International Corp., of Pittsburgh, PA, made the offer to the NYFD and it was well received. Currently the statue is in New York City at 8th Ave and 41st. It will be kept there permanently. It was the feeling of the Fire Fighters Memorial Foundation of MO, Board that it would be an honor to have our statue in New York City as a tribute to all those who served so well. Passers by are moved to tears and many visitors return to added flowers and candles around the statue daily. The Foundation hopes the statue will be part of a memorial to aid with the healing process for so many. It is also hoped it will serve as a reminder of the common bond that connects all who are a part of the fire service that serves this nation’s communities. The Foundation Board would like to thank all that has supported this effort and invites you to stop by and see the site. We will continue work at the site insuring we will truly have something that does justice to the memory of our fallen.
