While we are preparing for the upcoming dedication we look back on our efforts to remember fellow fire fighters that have left us this passed years. A number of firefighters will be remembered but please provide us information on any member of your department lost over the last year. We would like to get a photo, a paragraph about what they did for the fire service, and a paragraph about her/him as an individual. Your assistance will be required to fill out the attached form so that the information about the deceased will be accurate. DECEASED MEMBER GUIDELINES

The deceased member must have died in an "ACTIVE DUTY" status with a Fire Department in the State of Missouri, the interpretation will include: at work at the station, in response to, on an alarm, or returning from and includes, providing service (such as Public Relations). This will cover members of all fire departments to include a Department of Public Safety, where the individual was working as a fire fighter. If there are any questions as to an individual meeting the criteria, do not hesitate to give me a call.

The deceased's individual does not have to be a member of the Fire Fighters Association of Missouri to be placed on the Memorial Wall. Any and all fees will be waived for the departed individual, family and fire department.

Please take a few minutes and fill out the attached form as completely as possible and return in the self-addressed envelope.
